Naturally acquired plague most commonly manifests in the primary bubonic form, with fever and a painful swollen regional lymph node (bubo). Buboes develop most commonly in the inguinal region but also occur in axillary or cervical areas. Plague manifests less commonly in the primary septicemic form without localizing signs (fever, hypotension, purpuric skin lesions, intravascular coagulopathy, and organ failure) or as primary pneumonic plague (cough, fever, dyspnea, and hemoptysis) and rarely as meningitic, pharyngeal, cutaneous, ocular, or gastrointestinal plague.
