Fundamental to a functional medical home for children is an information system that supports a comprehensive, longitudinal record for each child and the ability to manage patient populations.1 The growing popularity of the Patient-Centered Medical Home (PCMH) program from the National Council for Quality Assurance2,3 formalizes one definition of the medical home and outlines the information management activities that a recognized practice should master.
