The Bright Futures Tool and Resource Kit, 2nd Edition contains the forms and handouts that health care professionals, as well as patients and families, need to perform the well-child visits and make them a productive and collaborative partnership to ensure the health and well-being of infants, children, youths, and families. The second edition builds on the original Toolkit’s system of integrated forms to streamline the well-child visit. This resource facilitates enhanced surveillance of patient and family health and well-being with an expanded Anticipatory Guidance section, extensively revised documentation, and educational handouts written in plain language to be clear and concise to patients and families.
This Toolkit helps ensure individualized care, giving opportunities for health care professionals to tailor care and anticipatory guidance.
Dedication
The Bright Futures Tool and Resource Kit, 2nd Edition, is dedicated to Paula Duncan, MD, FAAP. This edition of the tool and resource kit builds on her work as coeditor of its first edition and of Bright Futures: Guidelines for Health Supervision of Infants, Children, and Adolescents, third and fourth editions. Without Dr Duncan’s energy, insight, and spirit, Bright Futures would not have achieved relevance for current pediatric practice. She firmly believed that “the heart of Bright Futures is establishing trust to build a therapeutic relationship,” and she championed and devoted her career to the use of strength-based approaches. Dr Duncan’s warmth, joyfulness, and ability to see the best in people enabled her to behold the innate strengths of families. It was her passion to teach health care professionals how to see families as she did and to serve them better. This focus on strengths and protective factors in the clinical encounter of preventive services is her essential contribution.
